Application Submission Guidelines

SECTION I:

Signed Cover Sheet (Note:  Does not count against the 5-page limit). (Download Cover Sheet)

SECTION II:

Project Summary (Note:  Does not count against the 5-page limit).

Limited to one page, double-spaced, 1-inch margins, 12-point Times New Roman font. 

SECTION III:

Project Description (Maximum of five pages, double-spaced, 1 inch margins, 12-point Times New Roman. Font)

The Project Description must address the following categories:

• Sustainable
• Industry-Driven
• Replicable
• Measurable Outcomes
• Partnerships (if applicable)

SECTION IV:

Budget must contain the following items:  (Note:  Does not count against the 5-page limit).

• Spreadsheet w/Budget Categories (Download Budget form)
• Budget Narrative/Justification (Limited to two pages)
• Signed Conditions of Reimbursement Form (Hyperlink)

SECTION V:

Project Timeline and Schedule of Outcomes (limited to two pages)

SECTION VI:

Letters of Support

One letter of support is required for each partner listed in the proposal.  THIS SECTION APPLIES ONLY TO PROPOSALS SEEKING BONUS POINTS INDICATED UNDER CRITERIA FOR EVALUATION.
